1What is an Individual Number Notice?

An Individual Number Notice is a notice delivered to each resident to inform them of their Individual Number (My Number).

On the notice, the holder’s full name, date of birth, and Individual Number (My Number) are listed.
An Individual Number Notice cannot be used as proof of your Individual Number or as identification.

Note

Handling of Individual Number Notices for general identification (for companies)

An Individual Number Notice serves only to inform an individual of their Individual Number and cannot be used as proof of their Individual Number or as identification.
Please do not accept them for general identity confirmation procedures.

2Delivery of Individual Number Notices

Individual Number Notices will be delivered via simplified registered mail 2-3 weeks following registration of certificate of residence from May 25, 2020.

If you have registered your residence and yet have not received an Individual Number Notice, please consult your local municipal office.

*Your Individual Number Notice will be sent by simplified registered mail, so you must be present in order to receive it. In the event of your absence, you will receive a delivery notice with instructions on redelivery. Please make sure to receive it within the storage period.

This document notifies the resident of their Individual Number (My Number).

①Uni-Voice Code
You can scan the two-dimensional barcode (Uni-Voice code) with a compatible mobile phone or smartphone with the required app installed to listen to the information encoded in the Uni-Voice code.

【Uni-Voice coded message】
An Individual Number Notice that includes your 12-digit Individual Number (My Number) and an Individual Number Card application form are delivered from your local municipal office.
Your Individual Number is XXXX XXXX XXXX.
In addition to identity verification, provision/confirmation of your Individual Number is required for various procedures related to social security, taxation, and disaster response.
An Individual Number Card has many advantages, as it can be used as proof of your Individual Number, as official identification, and for various online applications. It’s also free, so we highly recommend you apply for one.

【Points to note】

  • To check the status of your application, your Application ID is required.
  • Make sure you provide a usable ID photograph.
  • Make sure you’ve filled out all of the required fields on the form, including the application date, and your name/signature.
  • Have you enclosed anything other than your Individual Number Card application form?

4Individual Number Notice intended use

An Individual Number Notice serves only to inform an individual of their Individual Number and cannot be used as general identification.

5If you lose your Individual Number Notice

If you lose your Individual Number Notice, it cannot be reissued.
In order to confirm your Individual Number (My Number), you will need to obtain an Individual Number Card, a copy of your certificate of residence with your Individual Number listed, or a Certificate of Items Stated in Resident Register.

*If you think you may have found someone’s Individual Number Notice, please hand it in at your nearest police station or police box.

About Individual Number Notices

1What is a notification card?

A notification card notifies each resident of his/her own Individual Number.

A notification card is a paper card which notifies every resident of My Number (the Individual Number).
Your "name," "address," "date of birth," "sex," and "My Number (the Individual Number)," etc., registered in the resident record of your municipality are noted on the card.

However, without an ID photo, the notification card must be used alongside other identification documents such as a drivers’ license or passport in order to confirm Individual Number and identity at the same time.

NOTE

About Using a Notification Card for General Identification (For Companies)

*While we are no longer issuing new Notification Cards as of May 25, 2020, you will still be able to use them to apply for an Individual Number Card.

*If you have a Notification Card and the information on the card such as your full name and address matches that recorded on your certificate of residence, you may continue using the card as proof of your Individual Number.
After May 25, if you have changed your name or address, you can show your Individual Number Card, a copy of your certificate of residence with your Individual Number listed, or a Certificate of Items Stated in Resident Register in order to provide proof of your Individual Number.

*If you have received a Notification Card and lost it, you will need to report this to your local municipal office.

*If you have received a Notification Card, you will need to return your Notification Card to your local municipal office when you receive your Individual Number Card.

● It is prohibited by law that a person other than those who are permitted by law copies the Individual Number. In addition, a person who falsified any matter stated on this card is punished by law.

Please tear it off and keep it until your Individual Number Card is issued.

By scanning the two-dimensional bar code, "Uni-Voice" with a cellular phone or an iPhone, etc., that provides a function for Uni-Voice, you can hear the information encoded in the Uni-Voice.

A notification card that includes your 12-digit Individual Number (My Number) and an application form for issuance of the Individual Number Card are sent from your municipality.
Your 12-digit Individual Number is XXXXXXXXXXXX.
From January 2016, in addition to identity verification, description/confirmation of your Individual Number will be required for various procedures related to social security, taxation, and disaster response.
Keep your notification card safe because it is a document which certifies your Individual Number.
In addition, it is really recommended to make an application for issuance of the Individual Number Card because it has a wide range of advantages other than certification of the Individual Number and the issuance fee will be free for the time being.

3Use of the notification card

These cards may be used when you are asked to provide your Individual Number (My Number) at an administrative office.
However, you are also required to present a document such as your driver’s license for identity verification.
In addition, if you obtain an Individual Number Card, you must return the Notification Card to your local municipal office.

*From May 25, 2020, Notification Cards were replaced by Individual Number Notices.
If you have a Notification Card and the information on the card such as your full name and address matches that recorded on your certificate of residence, you may continue using the cards as proof of your Individual Number.
After May 25, if you have changed your name or address, you can show your Individual Number Card, a copy of your certificate of residence with your Individual Number listed, or a Certificate of Items Stated in Resident Register in order to provide proof of your Individual Number.

4Loss of Notification Card

Notification Cards cannot be reissued if lost.
In order to confirm or verify your Individual Number (My Number), you will need to obtain an Individual Number Card, a copy of your certificate of residence with your Individual Number listed, or a Certificate of Items Stated in Resident Register.
If you have lost your Notification Card at home, please report it to your local municipal office.
If you have lost your Notification Card outside your home, please report it to the police.

*If you find a lost Notification Card, please bring it to your nearest police station or police box.
