Q1How can I apply for an Individual Number Card?
After the notification card and application form for issuance of the Individual Number Card are mailed by recorded delivery to the address on your resident record, make the application by mail or through on-line application on a computer or smartphone, etc.

Q2I lost or misplaced my Individual Number Card application form. How can I apply?
The application method will differ depending on your situation. Please carefully check the following.

*1. The Application ID is a 23-digit number that is used as the application form ID when you apply for your Individual Number Card.
A Notification Card
(Enclosed with Individual Number Notice or issued by your local municipal office)
It is written on the top right corner of the Individual Number Card Application Form/Electronic Certificate Application Form, which is sent together with your Individual Number Notice or Notification Card. (On the Notification Card, it is written below the box addressed to municipality, which is under the first dotted line.)
With this application ID, you can apply for your Individual Number Card online from your computer or smartphone. Scan the QR code on the application form with your smartphone’s camera and you can access the application website.
Please note that you can check the status of your application through the Application Status Inquiry Service using your Application ID and date of birth.
If you do not know your Application ID, we will not be able to check the status of your application. Please keep it in safe location until the issuance notice arrives.

- It takes 2 to 3 days from the date of receipt of your application form or from the date of online application to be able to check the status of your application. Please try again at a later date if you have applied recently.
- Application status can only be checked if the application was submitted within the past 6 months.
- You can check the status of your application from the time it is received to the time your card is sent out. After the card is sent to your municipality, it may take several weeks until you receive your issuance notice, as there are necessary procedures that must be completed prior to issuance. Therefore, after confirming the shipment of your card through this service, please refrain from contacting the municipality or visiting the municipality to pick up your card before you receive your issuance notice.

Q3What is the deadline to apply for an Individual Number Card?
There is no deadline for applying for an Individual Number Card.
Q4Can I still apply even if the address on the application is my old address?
You are able to apply even with an application form showing a previous address.
Even if your application form lists your previous address, your Individual Number Card will be issued with the address based on your most current residence registration information, if you have completed procedures for moving in or moving out before applying for your Individual Number Card.
The Issuance Notice will be mailed to your current registered address.
If you apply online, you may apply using the application form that you already have with no issues.
If you wish to apply by post and if the information on the application form and the certificate of residence are different, please cross out the incorrect information on the application form using a double line and write the correct information in the blank space of the form.
However, please note that your Issuance Notice or notice of incomplete application will be sent to your old address if you have not completed procedures to transfer your residency information to your new address.
Q5I have multiple application forms. Which form should I use?
Please use the most recent application form (※).
If there is no change in the address or name listed, applications received in the past can still be used.

Q6What should I do if I receive an Individual Number Card application form even though I have already applied?
We are resending the Individual Number Card application form with a QR code to those who do not yet have an Individual Number Card.
※We apologize if it was sent to who have already applied for and received an Individual Number Card by mistake.
Q7What is the Application ID on the Individual Number Card (My Number Card) application form?
When you apply for a My Number Card, the 23-digit number is used as the application form ID.

Q8What should I do if I don’t know my application form ID?
If you do not have an Individual Number Card application form with an Application ID, you can apply with a handwritten application form (which you can download below). Fill in your Individual Number on the form and mail it to us by post.
If you wish to apply online or if you do not know your Individual Number, you can obtain an application form for an Individual Number Card with your application form ID at your local municipal office free of charge.

※Please note that we are unable to provide your Application ID or Individual Number if you contact us by phone or chat.
Q9I don’t know my Individual Number (My Number).
Please confirm your number with your Notification Card or Individual Number Notice.
If you do not have your Notification Card or Individual Number Notice, please obtain a certificate of residence with your Individual Number printed on it at your municipal office.

Q10I received an application form. What should I do?

Q11Is applying for an Individual Number Card mandatory?
While applying for an Individual Number Card is not a mandatory requirement, we would like for as many people as possible to apply, as it is used to verify one’s Individual Number for certain administrative procedures and serves as a form of personal identification in-person and online, in addition to allowing users to use as their health insurance card or Convenience Store Issuance services using their electronic certificate, thereby enhancing the convenience of our residents.
Q12How can I check the status of my application?
You can check the status of your application through the Application Status Inquiry Service using your 23-digit Application ID and date of birth.
If you do not know your Application ID, please contact your local municipal office to check the status of your application.

Q13Can I apply at the municipality office?
In general, it is possible to do so. However, you may be required to bring certain items, such as your ID photograph.
We ask you to please contact the municipal office and ask in advance.
Q14Why use the application form ID?
We use a 23-digit application form ID to process the application without having to directly check residents’ Individual Number.
Even if someone sees your application form ID, no third party will be able to know your Individual Number, so please rest assured.
Q15The name and address on the re-issued notification card are hidden with asterisks, can I use this application form to apply?
You cannot use a re-issued notification on which the name and address are hidden with asterisks to apply.
If you wish to make an application, please use a new application form issued by your local municipal office or the Handwritten application form for issuance [English] or [Japanese].

Q17Can online applications be submitted via any PC or smart phone?
In principle, applications can be submitted via any PC or smartphone with the recommended operating environment.
We recommend the following operating systems and browsers for smooth operation of this website.
When using browsers other than the recommended ones or even when using the recommended browsers, the website may not be functional or display correctly depending on the user’s browser settings (e.g., JavaScript not enabled, etc.).
Additionally, please note that the website may not function well if you have installed certain software (e.g., browser extensions, etc.).

Q18May an application for the Individual Number Card be made even by children?
Individuals under the age of 15 and adult wards are required to have a legal representative file by proxy on their behalf.
Please fill in the name of the individual (under the age of 15) in the space for applicant’s name when applying for issuance and the full name of the legal representative in the Legal Representative field if filing by proxy.
Q19Can I apply for everyone in my household using a single e-mail address?
You can submit multiple applications with a single e-mail address. However, please note that all notifications regarding the applications will be sent to the same e-mail address.
Q20What should I do if I have completed my email address registration, but have not received the notification email or the authentication number email?
The following are possible reasons for not receiving the e-mails.
■ The registered email address registered is incorrect
Please register again with the correct email address.
■ Restrictions on receiving emails are turned on
If under your email settings, you have filter settings such as filters for specified domains, please enable emails sent from the following email addresses.
Please change the setting and apply again.
* Domain refers to the section following the “@” in the email address.

■ Emails are being sent to your spam folder
There is a possibility that the emails are being considered spam due to the filter settings of your email software or anti-virus software, or due to your provider’s anti-spam measures. Please check to see if the emails have been sent to your spam folder.
■ Your incoming mail folder/inbox has reached its capacity
Please check and adjust the capacity of your mailbox, and apply again.
■ You are rejecting e-mails containing URLs.
If you have a setting that does not allow you to receive e-mails containing URLs in the content, you may be unable to receive e-mails from this site.
Please change the setting and apply again.
Q21When making an application for issuance of the Individual Number Card, is it necessary to attach an ID photograph?
It is required to attach an ID photograph when making an application for the Individual Number Card.
Please write your full name and date of birth on the back of your ID photograph and paste it on the application form in the specified area.
The ID photograph must be taken within the last six months and include front view without a hat with a plain background.
For examples of identification photographs, please refer to the check points for photographs.
※ Please refrain from using too much glue when pasting the photographs and ensure that they have sufficiently dried up before sending your application form.
Q22Is there a size limit for ID photographs?

Q23Is there a file size limit when uploading ID photos for online applications?
Please note the following requirements for ID photographs.
- ・Must be photographed within the last 6 months (no hat, bust up, no backdrop)
- ・Must be photographed in a well-lit environment (Black and white photographs allowed )
- ・File Type: jpeg, jpg ※heic cannot be used
- ・Color Mode: RGB Color (other modes such as CMYK not accepted)
- ・File Size: 20KB – 7MB
- ・Pixel Size: 480 – 6000 pixel width, 480 – 6000 pixel height
Please note that images that have been edited using image editing software will not be accepted.

Q24How do I upload ID Photograph for my online application?
You can upload your picture on the ID photograph registration page by following the steps below.
- ①Click the red “Upload” button
- ②Select a photo from the “Photo Library”
- ③Click the light blue “Confirm” button
Q25An ID photograph needs to be 4.5 cm (H) x 3.5 cm (W). If I accidentally use a different size photo, will the card still be issued?
It depends on the resolution of the photo, the size of your face in the photo, and other factors. We cannot give you a general idea of how much error is acceptable, but if the photo can be trimmed, we will not consider it incomplete and proceed with the issuance process.
Although it is possible to trim the image to a certain extent, this does not mean that the image will be considered valid in all cases, so please apply using a photo of the specified size.
Passport size is a common size used for various certificates. In order to maintain the quality of the photograph of your face as an ID photo even after trimming, we have set the passport size as the standard size.
Q26What should I do if I cannot provide a photo, which fulfills the requirements, due to unavoidable reasons, for example, when the subject covers part of its head with material such as cloth for religious reasons or medical reasons as long as its face is clearly visible, the subject is an infant, a disabled person or bedridden, etc?
If you are unable to take an ID Photograph with the required standard due to unavoidable reasons, please notify us using one of the methods below.
Please prepare the ID Photograph that you have used or plan to use for your application to notify us.
<When notifying at your local municipal office>
Please inform the municipality in which you reside of the reason why it is not possible to take an ID Photograph with required standards and your Application ID.
<When notifying via mail>
Attach your ID Photograph and write the reason in the name column on the front of the Individual Number Card application form and submit your application.
<When notifying by phone>
Have the ID Photograph to be used for your application at hand and call the Individual Number Call Center (0120-95-0178, *Press 1 following the Voice Guidance). We will ask you to provide your Application ID, details regarding the ID Photograph, and the date of application (date you plan to submit your application) when you contact us.
*The local municipal office may inquire about your circumstances when issuing your Individual Number Card.
Please note that your application may be considered to be insufficient if the ID Photograph does not fulfill the requirements (e.g. if it is too dark, cannot be trimmed, etc.) or for reasons not relevant to the ID Photograph.
Q27How should ID photos for infants be taken?
Please be aware of the following points when taking facial photos of babies and preschool children.
- Please do your best to have your child facing the camera (photos where their face isn’t clearly visible are unacceptable.)
- Make sure your child’s eyes are open (unless your child has extenuating circumstances, such as a disability, photos of your child sleeping are unacceptable.)
- If your child is unable to sit on their own, please lay them down on a plain sheet and take their photo from above (photos with visible patterns in the background are unacceptable.)
- If you are holding your child in your arms, take care not to cover their face with your arm (photos where their silhouette, head, mouth, or any other feature are unclear, are unacceptable.)
In the event of extenuating circumstances where you are unable to take a photo that satisfies the requirements necessary for an application, please call the Individual Number Call Center toll-free number with your application ID number in hand before you apply. Individual assistance may be available. In the event that you are unable to call the Individual Number Call Center toll-free number directly, please speak to the respective help desk in the city, ward, town, or village in which you reside.
*There may be instances where the respective help desk in the city, ward, town, or village in which you reside asks you about your circumstances when issuing your Individual Number Card. Please note that your application may be sent back to you in the event that your facial photo is doesn’t meet the requirepments (a dark photo, photos too large to trim) or for another possible reason.
Please note that as of December 2, 2024, Individual Number Card for children under the age of 1 will not have photos displayed on their cards.
Q28Why do I have to write my name and date of birth on the back of my identification photograph?
We request that you write this information on the back of your photograph for identification purposes in the event that it comes loose from your application form. Please write your name and date of birth on the back of the photograph before affixing it to your application.
Q29Can I have my ID photograph taken at City Hall?
Depending on the municipality, it may or may not be possible to have your ID photograph taken at the municipal office. Please contact your local municipality.
Q30The Braille kana on my application is incorrect. What should I do?
The kana on the Individual Number Card application is only used with Braille. If you are not requesting Braille on your card, please apply as-is.
However, if you want your name written in Braille on your card, please contact your local municipal office to correct and re-issue your application form.
Q31I would like Braille to be displayed on my card, but I submitted an application without requesting Braille notation. Is it possible to correct this?
No corrections can be made to the application.
Please consult your local municipal office regarding the cancellation of the application and reapplying.
Q32I would like to have electronic certificate, but I submitted an application without requesting it. Is it possible to correct this?
No corrections can be made to the application.
Please inform the municipal office that you wish to have the electronic certificate upon receiving your Individual Number Card.
Q33What does “Information for Alternative Letters” on the application mean?
The information for alternative letters on the Individual Number Card is alternative kanji (Chinese characters) for those who have names using kanji that cannot be used for the Public Certification Service for Individuals.
Alternative letters will not be displayed on your actual Individual Number Card.
Q34Should I use the Gregorian or Japanese dating system when indicating the date on my application form?
Both the Gregorian and Japanese dating systems can be used.
Q35Is there a fee for applying for the Individual Number Card?
Currently, there is no fee.
Reissuance of your card is also available free of charge. You do not need to pay a fee when the card’s reissuance has been found to be unavoidable or when the card’s validity period has expired.
Q36What documents will be necessary to receive the Individual Number Card?

The necessary documents to receive the Individual Number Card are as follows;
- Issuance Notice
- Identity Verification Documents (within the period of validity)
One piece of identity verification document with an ID Photograph
Or other 2 identity verification documents (e.g., health insurance card and pension book)

If you are unable to prepare the documents required to receive the certificate, or if you would like to confirm what documents are appropriate for identification, please contact the municipal office of the municipality where you are registered as a resident.

Q37What is an Issuance Notice?
This is a postcard(*) sent to notify you that your Individual Number Card is ready to be issued.
Please bring your Issuance Notice, Notification Card, and identity verification documents when you come to pick up your Individual Number Card at the location detailed on your Issuance Notice.
*Formats such as A4 size documents may differ depending on the municipality.

Please check the location to pick up your Individual Number Card on your Issuance Notice.
Q40What should I do If I misplace my notice of issuance, which would inform me of the details pertaining to the collection of my Individual Number Card?
Please ask your municipality in which you have registered your residence to reissue the Issuance Notice, or check the required documents for when you do not have an Issuance Notice.
※Because the required documents differ by municipality, please contact your municipal where your residence is registered to confirm.
Q41How can I make an appointment to pick up my Individual Number Card? Can I also pick up my Individual Number Card on holidays or at night?
Individual Number Card is issued within the respective city, ward, town, or village in which you reside, so please speak to the respective help desk for more information.
In the event that you wish to have a proxy come and collect your Individual Number Card, please speak to the respective help desk in the city, ward, town, or village in which you reside and explain your circumstances.
Q42If I cannot obtain the identification documents required for the collection of Individual Number Cards, what should I do?
As personal identification documents are absolutely necessary during issuance of Individual Number Cards, please confirm with your local municipal office what alternative forms of ID can be used if you do not possess the personal identification documents during issuance specified on the Individual Number Card website.
Q43If I have applied for the Individual Number Card, but move and change municipalities, can I still receive my card at the municipal office?
If you move to a different municipality (ward, in the case of major cities designated by ordinance) before receiving your Individual Number Card, you will not be able to receive your card in the municipality you were living before you moved.
Therefore, please cancel your application, request a new application form from your new municipality and apply again.
Please note that if you have already completed procudures for moving out of the municpality, you will not be able to receive your Individual Number Card, even if you have the Issuance Notice.
*Generally, the Issuance Notice will be sent as a no-forwarding mail. If a request to forward mail has been submitted to the post office, your Issuance Notice will be returned to the issuing municipal office.
*Please check with your city or ward’s city hall about receving documents if you have moved across a ward which is part of an government-designated city.
Q44Can children collect their Individual Number Cards by themselves?
While Individual Number Cards will, as a principle, be handed directly to the person in question, a legal guardian must accompany individuals below the age of 15 or those considered to be adult wards.
Q45Is it possible to collect an Individual Number Card on behalf of someone else?
If you are unable to visit the location where your card will be issued to pick up the card in person due to unavoidable circumstances such as illness or physical disability, you may select a representative to pick up the card on your behalf.

Q46Why do I have to return my notification card to get an Individual Number Card?
Both your Individual Number and official identification are required for applications and procedures relating to insurance, taxation, and disaster prevention. An Individual Number Card fulfills both of these requirements.
On the other hand, a notification card can only be used to provide your Individual Number. If you have an Individual Number Card, you do not need a notification card.
As a result, by law you must return your notification card upon receipt of your Individual Number Card, where it will be safely disposed of by the local municipal government.
Q47Will I be able to continue to use my Basic Resident Registration Card?
You can use your Basic Resident Registration Card if it is still valid. However, as Basic Resident Registration Card cannot be newly issued, re-issued or renewed, please use the Individual Number Cards to apply.Although Basic Resident Registration Cards will not be newly issued, reissued nor renewed, it will be available for the remainder of their period of validity.
As of December 21st, 2018 (Heisei 30), the electronic certificates for the bearer’s signature for all Basic Resident Registration Cards have expired. If you wish to use an electronic certificate, please apply for an Individual Number Card.
In addition, by law, you are required to return your Basic Resident Registration Card upon receipt of your Individual Number Card. Note that you can request to keep your card as a memento following invalidation procedures at your local municipal office.
Q48Can I change the details in my Individual Number Card application?
If you have already submitted your application, you need to cancel your application. Please contact the municipality that you submitted your application to.
In the event that there is a change in the information on your application between when you filled it out and when you applied, please draw a double line through the section to be changed and write the correct information above it.
※We are unable to cancel your application if you call us or contact us by chat/email.
Q49Can I withdraw an application that has already been made?
Applications can be cancelled at local Individual Number Card counters. Please contact your nearest municipal office for details.
* It is possible to submit a new application after cancelling your original application by using the new application form for issuance issued by your local municipal office or by the Handwritten application form for issuance [English] or [Japanese].
Please note that an application cannot be returned after it has been received for processing.
※This matter is handled by individual municipal offices. We cannot cancel your application if you contact us by phone or chat.
Q50May I have an additional envelope for sending the application form(s)?

Q52When can I pick up my Individual Number Card?
About one month after applying for an Individual Number Card, an issuance notice will be delivered to your home address by your local municipal office.
Please check the contents of the notice for the earliest possible date when you can receive your My Number Card, as it differs from municipality to municipality.
※Issuance Notice may arrive in an envelope.
Please note that, after your Individual Number Card arrives at the municipality where your address is registered, it will take several weeks until you receive your issuance notice, as there are necessary procedures that must be completed prior to issuance. To reduce congestion at municipality offices, issuance notices will be sent after adjusting for shipping times.
Q53What should I do if I miss the deadline for picking up my card?
The card will be kept at the municipal office for a certain period of time (minimum 3 months), so please contact the municipality.
After the storage period has expired, your card will be disposed of for security reasons. In that case, you must pick up a new application form at your local municipal office and reapply.
Q54What parts of the issuance notice should I fill out?
If you are coming to pick up the card in person, please write your name and address, and put your signature in the “回答書” section.

Q56Am I required to set a PIN number even if I do not need an electronic certificate?
Even if you do not request an electronic certificate, you are required to set a PIN number for the Basic Resident Registration and Card Info Input Support applications.
Q57Can passwords with numbers only be used for the electronic certificate for the bearer’s signature?
For your password ,capital alphabets from A to Z, numbers from 0 to 9 can be used and and at least one of each are required.
※6 alphanumeric characters or more but not exceeding 16.
Q58Will my Individual Number Card password be made known to the staff at the municipal office?
As a rule, you must input your Individual Number Card PIN by yourself, so staff at the municipal office will not typically be informed of your PIN.
Please note that in the case of applications and procedures submitted by a representative other than a legal guardian (parent, adult guardian, etc.), municipal office staff must input the PIN on the individual’s behalf. However, all staff are bound by an oath of confidentiality. Furthermore, in the case that a staff knew your Individual Number, it would be impossible for him or her to use it without your Individual Number Card, so please rest assured that it will not be misused.
Q59How do I apply by mail?
Please fill in the necessary information and send the application form with your ID photograph attached.
If you use the envelope for sending the application form, you can apply without affixing a stamp.
If you are using a handwritten application form, please be sure to check that you have filled it out correctly, as it will take longer to issue your card if there are mistakes or missing items.

Q60Can I enclose several application forms in the return envelop? Also, can I enclosed application forms for non-family members?
Yes, you may enclose several application forms. However, the weight should not exceed 50g. You may also enclose the application forms of non-family members.
Q61Can an overseas resident apply?
As of May 27, 2024, Japanese citizens who live overseas and did not possess an Individual Number Card (limited to those who departed from Japan on or after October 5th, 2015) are now able to apply for one.

Q62What documents do I need to bring to apply at the municipal office?
If you plan to apply at the municipal office, please check the website of your municipality or contact them directly.
Q63Can I apply for an Individual Number Card on someone else’s behalf?
In principle, the applicant must apply for an Individual Number Card themselves.
However, for those under 15 years of age or an adult ward, an application by a legal representative is required.
In the event you are facing difficulty in filling out your Individual Number Card application, applications will still be considered valid if you have a caregiver or help desk worker fill it out on your behalf, and then you, the applicant, sign the application with your personal seal.
In the event that it is recognized that the applicant faces hardship in receiving their issued Individual Number Card, it is possible to have a proxy come and accept it in their place. Please speak to the respective help desk in the city, ward, town, or village in which you reside for more information.
Q64What kind of background should I use for my ID Photograph?
Please use an ID Photograph with a plain background to apply. For details, please refer to the check points for ID Photographs.
Q65If I am temporarily staying in a different municipality for work or education, where can I pick up my Individual Number Card?
You must pick up your card at the municipality in which you have registered your residence.
It is not possible to receive an Individual Number Card at a temporary residence. In general, the Individual Number Card is issued to the applicant him/herself. If you would like a representative to receive your Individual Number Card, please contact the municipal office where your residence is registered and consult them your current situation.
Q66I received a notice of incomplete application. What should I do?

The email notifying you of the error will include the details of the error and the URL to reapply. If you follow the instructions and reapply, your application will be processed for card issuance.
<Application by mail>
We will send you the original application form with the error, notice of incomplete/Individual Number Card reapplication form (which states the details of the error), and a return envelope.
After checking the details of the error, please reapply using the reapplication form and envelope, and it will be processed for card issuance.
Q67What should I do if I applied more than once?
In the case of a duplicate application, the first application will be used if there is no error in the contents of your application. If you wish to cancel your first application, please contact your local municipal office.

About the Validity Period of Individual Number Cards and Electronic Certificates
Q1What should I do when I receive the renewal notice (Notification of Validation Period) for my Individual Number Card?
If the enclosed application form contains an application ID, you can apply via a computer or a smartphone, via a public ID photo booth or via post.
If the enclosed application form does not contain an application ID, the application can only be submitted by post.
Please fill out the form with necessary information including your 12-digit Individual Number, attach your ID photograph and send only the application form back using the enclosed return envelope.
Q2My card’s validity period expired before I received my new card. Can I still use it as my Individual Number Card?
If the validity period of your card has lapsed, you can no longer use it as an identity document or use an electronic certificate.
You will need to bring your expired Individual Number Card when receiving the new card. Please keep it in a safe place until you can receive your new card.
Q3What should I do when I receive “the renewal notice (Notification of Validation Period) for electronic certificate”?
To renew your card, please bring the validation period notification and your Individual Number Card to your local municipal office before the expiration date.
(It is possible to renew the card even if the expiration date has passed. For further information regarding the procedure, please contact your local municipality.)

Q4Can I renew my electronic certificate online?
We are sorry, but online renewal is not available. In order to ensure the security of electronic certificates, your identity must be verified in person before your certificate can be issued. Therefore, we ask that you go to your municipal office to renew your certificate.
Q5Is it optional to renew my electronic certificate (for my Individual Number Card)?
Yes, it is optional, but you will not be able to use services such as e-tax or Convenience Store Issuance.
However, you may still use the card as a form of identity verification until the expiration date.
Q6When can I renew my electronic certificate?
Renewal applications can be accepted starting three months prior to the date of expiration. There are instances where your city, ward, town, or village may require a prior reservation. Q2What should I do if my Individual Number Notice contains incorrect information, a spelling error, or a typo?
Please contact your local municipal office to correct the information on your certificate of residence.
Q3I’m living overseas. Will I receive an Individual Number Notice?
Those without a certificate of residence in Japan, including those residing outside Japan, are not issued Individual Numbers (My Numbers), and therefore do not receive Individual Number Notices. If you move to Japan and register your residence, you will be issued an Individual Number Notice.
Q4Do I receive an Individual Number Notice if I’m not a Japanese citizen?
Even residents of other countries receive Individual Number Notices if they move to Japan and register their residence.
Q5If I have a new baby, does the baby need to be issued an Individual Number (My Number)?
Once you register the birth and your baby’s residence, an Individual Number Notice will be generated and an Individual Number Notice provided, so there is no need to apply for one separately.
Q6Does my Individual Number change if I change residence?
Unless your Individual Number is divulged and there is a risk of misuse, it will never be changed, and you will continue to use the same number throughout your life.
Q7Can I use an Individual Number Notice as an identity verification document?
The Individual Number Notice serves only to inform one’s Individual Number and cannot be used as identity verification document.
Q8If I lose the Individual Number Notice, can it be reissued?
An Individual Number Notice cannot be reissued.
In order to confirm or verify your Individual Number (My Number), you will need to obtain an Individual Number Card, a copy of your certificate of residence with your Individual Number listed, or a Certificate of Items Stated in Resident Register.
Q9If I am unable to change my certificate of residence due to special circumstances such as stalking or domestic violence, how should I receive my Individual Number Notice and Individual Number Card?
If you are unable to have your Individual Number Notice and Individual Number Card issue notice delivered to the address where your certificate of residence is registered due to special circumstances, you may register your current address at that municipal office and have the documents delivered there.
Please consult the municipal office where your residence is registered about the process for registering your current address.
Q10When Will My Individual Number Notification Come?
Your Individual Number Notification will be delivered by registered mail approximately two to three weeks after registering your address.
If you are not home when a delivery attempt is made, a missed delivery notice will be left in your mailbox. Please follow the directions on it and reschedule your delivery before the holding deadline.
In the event that the holding deadline has passed and your mail has been returned to your city, ward, town, or village, or if you still haven’t received it, please go and speak to the help desk at your respective city hall.
*Your Individual Number Notification is sent via mail which cannot be forwarded. Even if you move and set up mail forwarding to a new address, it will not be forwarded. Instead, it will be returned to the city hall help desk which originally issued it.
About Notification Cards
Q1Who issues notification cards?
The mayor of a municipality that has the person’s resident record.
In addition, the Japan Agency for Local Authority Information Systems (J-LIS) will issue notification cards with the authority from mayors throughout Japan.

Q2What should I do with my Notification Card now?
Notification Cards already issued can be used to confirm the holder’s Individual Number, so please keep it.

Q3Why are Notification Cards being discontinued?
This change was necessary as residents and municipal offices who found the procedures for changing the address on the Notification Card when moving to be an inconvenience, and was also motivated by a desire to hasten the switch from paper cards to Individual Number Cards with electronic certificates serving as official verification of the holder’s identity.
Even after Notification Cards are discontinued, you will still be able to use them to apply for an Individual Number Card. Please check here for details.
Q4Will the Number be changed when I transfer my resident record?
Unless the number is leaked and there is a risk of illegal use, the Number will never be changed because the same Number will continue to be used throughout the bearer’s lifetime.
Q5Can a Notification Card be used as an identity verification document?
Notification cards can only be used for confirming Individual Number, so they cannot be used as an identity verification document.
Q6Is reissuance of the notification card acceptable?
If it is acceptable, where and how do I request for it?
Notification Cards cannot be reissued.
Beginning May 25, 2020, residents are issued Individual Number Notices as notification of their Individual Numbers in place of Notification Cards.
In order to confirm or verify your Individual Number (My Number), you will need to obtain an Individual Number Card, a copy of your certificate of residence with your Individual Number listed, or a Certificate of Items Stated in Resident Register.
Q7What should I do if my name or address has changed and the information on my Notification Card is out of date?
After May 25th, 2020 (Reiwa 2), the procedure for changing items on the notification card cannot be performed, including the name, address, etc. written on the card.
Q8If the name or address on my Notification Card is out of date, can I throw away the Notification Card?
As your Notification Card is required to confirm your Individual Number when applying for an Individual Number Card by mail (handwritten application), please keep it.

You can check which municipalities provide this service on the Convenience Store Issuance service page. A 4-digit PIN for the electronic certificate for user certification is required to use this service.
Q6What’s an electronic certificate?
An electronic certificate is a digital certificate in which a reliable third party (certificate authority) proves the identity of the holder, and can replace the use of a personal seal in paper-based transactions.
There are two types of electronic certificates that can be used with an Individual Number Card.

Q7What is the PIN of the electronic certificate for user identification?
This is the 4-digit PIN code that you set when you set up the electronic certificate for user identification. It is a certificate and PIN that you can use to prove that you are the user when using the internet, such as when using the public personal identification service.
In some cases, it may be provided to you with the Request for PIN Setup form (Example 1), however the format may be different depending on the municipality.
If you have forgotten your pin or have locked your card by accidentally entering the wrong PIN three times, you need to reset your PIN at the municipal office that you registered your residency with.
Q8What is the PIN of the “App to support entering items on the card”?
It is the PIN that you set when your Individual Number Card was issued. It is a 4-digit number configured to read the data of your Individual Number and 4 items of personal information (name, address, birthday, and sex).
In some cases, you may be given a PIN Setting Slip (Example 1) or Request for PIN Setup form (Example 2), however the format may be different depending on the municipality.
If you have forgotten your pin or have locked your card by accidentally entering the wrong PIN three times, you need to reset your PIN at the municipal office that you registered your residency with.
Q9My Individual Number Card is not being read correctly during e-Tax application process. What should I do?
Please perform a validity check of your electronic certificate and test if your Individual Number Card can be read correctly.
If it is valid, please contact the e-Tax service office as the problem is not related to the reading of your electronic certificate or Individual Number Card.

Q14What should I do if my Individual Number Card is damaged and the IC chip does not function?

Q19What to Do When Moving Outside of Japan?
As of May 27, 2024, Individual Number Card may still continuously be used even after you live overseas.
If you wish to continue to use your Individual Number Card after moving overseas, please take your Individual Number Card with you when you fill out your departure procedures for leaving Japan and do any additional procedures required to allow continued use.

Your information on your Individual Number Card will be displayed exactly as it is on your Certificate of Residence.
Once your Certificate of Residence includes your maiden name (former surname), apply for your Individual Number Card and the card will have your maiden name (former surname) listed on it.

Q1What are facial recognition Individual Number Card?
These are Individual Number Card in which the identification method is limited to facial or visual recognition, eliminating the need to set a PIN.
Q2What can I do with a facial recognition Individual Number Card?
It is primarily envisioned for use as a health insurance card. After confirming the identity of the applicant through facial or visual recognition, the system will allow online eligibility verification, and with the consent of the individual, it will also enable access to information such as specific health checkups, medical information, and medication information. In addition to this, the photo and four pieces of information (name, address, date of birth, and gender) on the face of the card allows it to be used as an identity verification document.
Q3Is there anything that can’t be done using a facial recognition Individual Number Card?
A facial recognition Individual Number Card cannot be used for services that require the entry of a PIN, such as logging into Mynaportal, issuing of documents at convenience stores, and various online procedures.

If you already have an Individual Number Card, you can bring it to your local municipal office personally and have it switched to a facial recognition Individual Number Card right away. If a proxy is performing the procedures, a power of attorney is required to complete the procedure.
For those applying for a new Individual Number Card, it will take approximately one month to receive it, the same as a regular Individual Number Card.

- If a proxy is coming to the municipal office to pick up a newly applied for Individual Number Card, ensure that the “Do not set any PIN” check box is checked in the PIN setting section of the issuance notice sent by the municipality, affix a privacy sticker, and have the proxy bring the card and necessary documents to the municipal office in your municipality.
- If there is no check box for “Do not set any PIN” on the issuance notice you received, please leave the PIN field blank and write in the margin that you do not wish to set a PIN, and have the proxy bring it to the municipal office in your municipality.
- In the event that a person who already has an Individual Number Card requests a proxy to switch their card to a facial recognition Individual Number Card, a letter of attorney signed by or affixed with the name and seal of the cardholder is required.
Q15In the event that a person aged 15–17 wishes to switch to a facial recognition Individual Number Card, can their legal representative make an application without a letter of attorney?
In the event that a person aged 15–17 wishes to switch to a facial recognition Individual Number Card, this must be done at the cardholder’s own volition. A legal representative may not declare such intention on their behalf.
Furthermore, if a legal representative is to perform the procedures, a letter of attorney from the cardholder is required. Please inquire with your local municipality regarding the documents required for the proxy application process.
Q16Can an application be made if the cardholder is unable to fill out a letter of attorney?
If, despite it being difficult for the cardholder to write a letter themselves, they are able to express their intention by affixing a personal seal, the letter itself may be written on their behalf.
Q17What happens to the Electronic Certificate of the Bearer’s Signature already associated with my Individual Number Card?
A facial recognition Individual Number Card cannot be associated with an Electronic Certificate of the Bearer’s Signature.
This is because the entry of a 6- to 16-digit PIN when using an Electronic Certificate of the Bearer’s Signature cannot be substituted by facial recognition or visual inspection.
If your Individual Number Card is associated with an Electronic Certificate of the Bearer’s Signature, the certificate will be invalidated upon switching to a facial recognition Individual Number Card.
Q18Can a facial recognition Individual Number Card be set up without associating it with an Electronic Certificate for User Identification?
Because a facial recognition Individual Number Card does not require the setting of a PIN, and the method of cardholder identification when using the Electronic Certificate for User Identification associated with the card is limited to machine facial recognition or visual confirmation, it must be associated with an Electronic Certificate for User Identification.
Q19How should I apply to use a facial recognition Individual Number Card as a Health Insurance Card?
To use a facial recognition Individual Number Card as a Health Insurance Card, you must apply for Health Insurance Card use.

Q21How does the facial recognition Individual Number Card differ from the Certificate of Eligibility that can be used in place of a Health Insurance Card?
With a facial recognition Individual Number Card registered for Health Insurance Card use, the cardholder’s identity can be reliably verified using the electronic certificate of the Individual Number Card through face recognition or visual confirmation.
While the Certificate of Eligibility cannot provide past medication information to medical institutions and pharmacies, the facial recognition Individual Number Card can do so, allowing for more appropriate medical care.
Q22If I switch to a facial recognition Individual Number Card, can I check my medical visit history and medication information outside of Mynaportal?
There is no way to check your medical or medication information other than through Mynaportal.
Please note that medical and medication information can be provided to medical institutions with the consent of the cardholder.
Q23Is it possible to use a facial recognition Individual Number Card to complete the special notice of moving out/moving in procedures?
A facial recognition Individual Number Card can be used to complete the special notice of moving out/moving in procedures, but it cannot be used for online moving out/moving in reservation via Mynaportal.
