“Notification card” can be separated along the micro perforation line.
Please fold several times and tear along the perforation line and keep the card safe.

If there is any error in the name, address, etc. printed on the form or there is any change due to moving, etc., please contact your local municipality as you will not be able to use this application form.

It takes about one month*2from the application for Individual Number Card until the shipment of Notice of Issuance*1by municipalities.

1What is an Individual Number Card?

An Individual Number Card is a card with an ID photograph in which My Number (the Individual Number) is described.

An Individual Number Card is a plastic card which contains an IC chip, and the bearer’s name, address, date of birth, sex, My Number (the Individual Number), and ID photograph, etc., are displayed on it.
This card can be used as an identification card for identity verification as well as to receive a wide range of services including municipality services and electronic applications using the electronic certificate such as e-Tax.

What an Individual Card can do

As a document which certifies the Individual Number
You can use it as a document which certifies My Number (the Individual Number) in various situations where required to present My Number (the Individual Number).

On-line application for various administrative procedures

2How to make an application for your Individual Number Card

You may receive your Individual Number Card from January 2016 by making the application using an application form for issuance of the Individual Number Card that you will receive together with the notification card. The application methods are as follows;

(1) Application by mail
Attach your ID photograph to the application form for the Individual Number Card and then enclose the form in a return envelope and send it by mail.

(2) Application via the website using a smartphone.
Take your ID photograph using a smartphone and make an on-line application from the prescribed form.

* Other methods are also under consideration.

(3) Application by a personal computer
Take your ID photograph with a digital camera and access to the website for application using a designated application form.

(4) Application from town ID photo booth
Bring an application form and take an ID photograph by ID photo booth which is approved for application
*Some town ID photo booths cannot be used for the application

3How to receive your Individual Number Card

As a rule, you may receive the card at your municipality office.
After you make an application for issuance of the Individual Number Card, your municipality will send a notice of issuance to you. You may receive your Individual Number Card by bringing the notice of issuance, the notification card and your identity verification document (*) to the place of issuance described in the notice of issuance.
In addition, if you have your Basic Resident Registration Card, please bring it to the counter of issuance together with the notice of issuance and the notification card.

 

* Identity verification documents

  • One of the following items: a Basic Resident Registration Card, a driver’s license, a certificate of driving history (only the one whose issuance date is on or after April 1, 2012), a passport, a physical disability certificate, a mental disability certificate, a rehabilitation certificate, a resident card, a special permanent resident certificate a landing permit for temporary refuge, or a permit for provisional stay
  • If you do not have any of the above, two documents including either your "full name and date of birth" or your "full name and address", and considered as appropriate by the mayor.
  • (Example) A health insurance card, a pension book, an employee identification card, a student identification card, various documents in which the school name is described, a bankbook, a Claimant Certification for Medical Care

4Loss of Individual Number Card

Please complete procedures to terminate functionality of a lost Individual Number Card in order to prevent possible misuse.

Functionality can be terminated by contacting the Call Center for the Social Security and Tax Number System by phone or fax.

*Fax is only accepted from the hearing impaired.

Inquire

Procedures for re-issuance can be completed following functionality termination at your local municipal office, but they require a receipt number copy from a police station or police box.

*Receipt Number: a number provided to you by a police station or police box when you report a lost article.

Bring the copy of your receipt number to your local municipal office and conduct re-issuance procedures.

You can reactivate functionality if you find your Individual Number Card.

1What is a notification card?

A notification card notifies each resident of his/her own Individual Number.

A notification card is a paper card which notifies every resident of My Number (the Individual Number).
Your "name," "address," "date of birth," "sex," and "My Number (the Individual Number)," etc., registered in the resident record of your municipality are noted on the card.

However, without an ID photo, the notification card must be used alongside other identification documents such as a drivers’ license or passport in order to confirm Individual Number and identity at the same time.

NOTE

About Using a Notification Card for General Identification (For Companies)

Notification cards can be used to confirm Individual Numbers. Please do not accept them for general identity confirmation procedures.

Your Notification Card will be delivered to you by registered mail after you register your Resident Record at your local municipal office.

Note that it takes around 3 weeks to deliver Notification Cards for newborns and people moving to Japan from overseas.

Please contact your local municipal office if you have already registerd your Resident Record but have not yet received a Notification Card.

*Your notification card will be sent by registered mail, so you must be present in order to receive it. In the event of your absence, you will receive a delivery notice with instructions on redelivery. Please make sure to receive it within the storage period, or else it will be returned to the sender.

Your 12-digit Individual Number is XXXXXXXXXXXX.
From January 2016, in addition to identity verification, description/confirmation of your Individual Number will be required for various procedures related to social security, taxation, and disaster response.
Keep your notification card safe because it is a document which certifies your Individual Number.
In addition, it is really recommended to make an application for issuance of the Individual Number Card because it has a wide range of advantages other than certification of the Individual Number and the issuance fee will be free for the time being.

Receive Notice

Your Notice of Issuance (postcard) arrives by post.

 

Go to the Issuance Office

After confirming your identity, you will be asked to set a PIN.

  • An application is required to receive your Individual Number Card.
    You can apply via the application form included with your Notification Card, online, or at a local ID photo booth*.

*Some ID photo booths do not support applications.

Important Note

  • To submit an application after relocation, please request a new application form from your local municipal office when submitting your address change documents, or download a form from here.

     

  • After submitting your application, there is a 3-week processing period until your card is created.
    After this, your card will be sent through the postal service to your local municipal office, which will handle any final issuance procedures and send you a notice of issuance by mail. Please wait until you receive this notice.

Important Note

  • If you move to a different municipality before receiving your notice of issuance, you must reapply. Please complete procedures for reapplication when you are registering your address at your new municipal office.

*In the case of government designated ordinance cities, it is outside the area

  • After receiving your notice of issuance, you yourself should visit the service counter by the date described on the issuance notification with required things.

*Only if you are not able to visit the place to receive your card due to your sickness, physical disability, or other unavoidable reasons, your representative can receive your card.
